Towards a new effective ICT-based Care Model for chronic diseases in Europe
The RICHARD project intends to define and implement innovative models for the management of chronic conditions based on Information and Communication Technology (ICT) in four different European Regions.
The scope of RICHARD is the definition of new scenarios of care and the design of innovative ICT based territorial clinical pathways that, through the use of technology and the integration of resources and capabilities of the territory, enables patients to be supported outside the hospital, guaranteeing a better quality of life.
The challenges are “…to identify, enhance and even design new paradigms ICT based for the management of chronic diseases consistently with the potential and the needs of each regional territory.” (Wagner extended chronic care model 1996).
